Policies and Procedures

ASSUMPTION OF RISK
All participants are expected to recognize and accept the inherent risks of their activity. 深夜福利站 does not accept responsibility for injuries sustained by persons participating in recreational activities. Each participant is personally responsible for obtaining and maintaining coverage through the Ontario Health Insurance Plan (OHIP) and/or other agencies or plans.

Campus Recreation and 深夜福利站 are NOT responsible for the loss or damage of personal belongings left in lockers or throughout the facility.

EQUIPMENT DESK RENTALS
The WSRC offers rental equipment for participants to use in our facility. Rental items will be signed out on the participant's account by scanning their membership card. Equipment must be returned the same business day. If equipment is not returned within seven (7) days, the participant's account may be subject to a suspension. Participants borrowing equipment should be aware that they are responsible for lost or damaged items. In the event, equipment is returned damaged or lost, set rates will be applied.

MEMBERSHIP, LOCKER & TOWEL SERVICE REFUNDS
A 10% administration fee* will be applied to all refunds. All refunds will be pro-rated based on time used. Since our annual memberships and services are discounted substantially, refunds will only be issued during the first nine (9) months of the membership/service. Memberships will not be refunded within 7 days of the expiry day.  Refunds will not be issued on one (1) month memberships.

Payroll Deduction Cancellations: Requests for payroll deduction cancellations must be made, in writing, to Membership Services. A 10% cancellation fee will apply.

On occasion, certain facilities may not be available due to maintenance, special events, etc. Campus Recreation will provide notice whenever possible, but memberships will not be extended.

MEMBERSHIP HOLD POLICY
Membership holds are only available for annual memberships.  We will gladly honour membership hold requests for medical reasons, academic sabbaticals, or temporary employment leaves. Hold requests must be submitted in writing (or by email) to Membership Services and must be for a specific time period.

Holds will not be processed retroactively. Members may request a membership hold once per year for a minimum of one month up to a maximum of six months.

COURSE REFUND POLICY
There will be a 10% administration fee* for all refunds, except if a course is cancelled. In addition to this fee, the following conditions will apply:

Up 48 business hours prior to the 2nd class beginning, course fees refunded, minus the value of the first class (EXCEPTIONS: Once a weekend crash course or any Aquatic Leadership course has started, there is no refund).
Under all other circumstances, partial refunds will be issued for medical reasons only (with medical documentation).

Sports & Recreation Services reserves the right to cancel a course for any reason; in this case a full refund will be issued with no administration charge.

* The 10% administration fee may be waived if transferring to another course/program.

INTRAMURAL REFUND POLICY
Intramural registration fee(s) will be refunded, minus a 10% administration fee*, prior to the Registration Close Date. After the Registration Close Date, refunds will be issued for medical reasons only (with medical documentation).

* The 10% administration fee may be waived if transferring to another league/program.

SPORTS CLUBS MEMBERSHIP REFUND POLICY
All Club memberships will be refunded within 14 days of purchase, minus a 10% administration fee*. After this 14-day trial period, partial refunds will be issued for medical reasons only (with written documentation).

* The 10% administration fee may be waived if transferring to another Club/program.


PERSONAL TRAINING REFUND POLICY
Refunds will be issued for medical reasons only. Clients must submit their refund request, along with medical documentation, to the Program Coordinator, Fitness & Wellness. All refunds are subject to a 15% administration fee.

WELLNESS SERVICES CANCELLATION POLICY
A minimum of 48 hours' notice is required to change or cancel an appointment. Failure to provide 48 hours' notice and/or show up for a scheduled appointment will result in a forfeiture of the appointment.

MEMBERSHIP EXTENSIONS
On occasion, certain facilities may not be available for members' use due to special events or maintenance. Advance notice will be posted whenever possible; memberships will not be extended. Membership refunds/extensions will not be issued for maintenance periods.

FITNESS CENTRE POLICIES
WESTERN ID/MEMBERSHIP CARD
Members must present their 深夜福利站 ONECard or WSRC Membership card to enter the facility.  Members who have forgotten their 深夜福利站 ONECard or WSRC Membership card are allowed 2 free "look ups" per term (September-December, January-April and May-August). Members will be charged $3 for all subsequent look ups, which will reset at the start of every term.

Members who use other individual's cards or who do not present personal identification cards to enter the facility, will be suspended from the facility.

WESTERN SPORTS & RECREATION CONCUSSION CODE OF CONDUCT 

深夜福利站 Campus Recreation is committed to the safety and well-being of the participants in our programs. Participation in sport programs and activities includes a risk of injury outlined in our Policies and Procedures page. In recognition of the potential seriousness of any and all injuries, ALL participants in Campus Recreation sanctioned programs are required to read and verify that they will adhere to the Policies and Procedures, including any and all Code of Conducts listed below. This is a mandatory condition prior to participation in any sanctioned activity. 

   • 深夜福利站's Sport and Recreation Athlete Concussion Code of Conduct

Participation in sport programs and activities includes risk of injury that includes concussions. Concussions can pose a serious health risk with short and long term consequences if not recognized and handled appropriately. Sport Leaders have a significant role to play in concussion identification and management. In recognition of the potential seriousness of a concussion, ALL participants in 深夜福利站’s Sport and Recreation Services sanctioned programs are required to read and verify that they will adhere to the 深夜福利站’s Sport and Recreation Services Athlete Concussion Code of Conduct, and any other policies provided through this page, as a mandatory condition prior to participation in any sanctioned activity.

I commit to following the concussion related protocols and expectations as highlighted below:

RECOGNIZE

•  I understand that a concussion is a serious head injury that can have short and long-term effects.
•  I understand that any blow to the head, face, neck or body may cause jarring of the head and a concussion.
•  I understand that concussions can’t be seen, I might notice signs immediately or symptoms may occur hours or days afterwards.

REPORTING, REMOVAL FROM ACTIVITY & RETURN TO ACTIVITY

•  I will be honest and forthcoming about any signs and symptoms related to a concussion that I may be experiencing and report any injury to my Sport Leader.
•  I will report to my Sport Leader if another individual reveals they are experiencing concussion-like symptoms, or if I suspect another individual has sustained a concussion.
•  I understand I will not be able to return to play if I exhibit any signs/symptoms of a concussion and need to be cleared by a physician or qualified medical professional prior to returning to sport.
•  I commit to the return-to-sport process and established protocols.

 RESPONSIBILITY

•  I will respect the roles and responsibilities of all Sport Leaders in “Removal from Sport” and “Return to Learn” and “Return to Sport” protocols.
•  I will properly wear the appropriate equipment for my activity at all times; and understand that equipment such as helmets and mouth guards may help, but are not guaranteed to prevent concussions.
•  I will help prevent concussions by committing to fair play and respect for all; respecting other athletes, coaches, team trainers and officials.
•  I commit to zero-tolerance for play that is considered high-risk for causing concussions.
•  I understand that engaging in prohibited play, with high concussion risk, will be subject to mandatory expulsion from play; and that these consequences will increase with subsequent offences.

ACKNOWLEDGEMENTS

•  I have reviewed and commit to this 深夜福利站 Sports & Recreation Athlete Concussion Code of Conduct.
•  I understand that concussion education is a mandatory requirement of my participating in any 深夜福利站’s Sport and Recreation Services sanctioned activity AND commit to reviewing the mandated 深夜福利站’s Sport and Recreation Services Concussion Education Resources prior to participating in any 深夜福利站’s Sport and Recreation Services sanctioned team/club and/or activities.
•  I understand that participation in sport and activity includes a risk of injury that includes concussion. I will not hold 深夜福利站 or its representatives responsible in the event of injury.

Definitions

“Concussion” is a brain injury. It can not be detected by X-ray, CT scan or MRI. It may affect the way a person thinks, feels and acts.

“Sport Leader” is any coach, instructor, official, therapist, doctor, administrator and/or other individual who has responsibility for the organization of the sport activity.

“Return to learn/sport” are medically supervised processes/protocols that gradually introduce the injured athlete back to academics/activity to ensure they are fully recovered before they are exposed to full mental or physical tasks.
